According to Fall 2013 data from the Leapfrog Hospital Safety Score, over 440,000 patients are dying annually from preventable hospital errors. Leapfrog assigns letter grades A-F to more than 2,500 general hospitals across the country. While hospitals are making some progress in terms of patient safety, it is too slow. Some hospitals have improved by one or two letter grades, the vast majority have not shown any improvement in the new data. The best way to combat these errors is to educate patients, so they can demand action and attention.
